AOS State Liaison Agreement
Before proceeding to our State Liaison Volunteer Form, please review the terms of becoming an AOS liaison, then use the links at the bottom of the page to indicate your agreement and move on to the actual form:
AGREEMENT
As of , the effective date of this agreement, the American Optometric Society
(“AOS”) and you (“Liaison”) do hereby agree as follows:
WHEREAS, the AOS is a national organization dedicated to the representation of
optometrists and the advancement of their interests in the profession of optometry; and
WHEREAS, AOS, in order to more effectively advance its purposes, goals, and mission
believe it to be in the best interest of the AOS to engage the assistance of individuals throughout the
United States as volunteers to help the AOS in the capacity of state liaison; and
WHEREAS, Liaison desires to serve as a volunteer state liaison for the AOS;
NOW, THEREFORE, AOS and Liaison agree as follows:
- Liaison is a volunteer, and shall not be deemed or held out as an employee or agent,
or to otherwise have any paid or legal relationship with the AOS. Liaison agrees that, as a volunteer,
he/she shall not seek or expect payment from AOS for services rendered, and is in no way employed
by the AOS and AOS does not assume responsibility for the acts or omissions of Liaison. - Liaison, when serving in his/her capacity as a liaison for the AOS, agrees that he/she
will comply at all times under all circumstances with the laws, rules, and regulations of the United
States and of the state and jurisdiction in which the Liaison is acting. - Liaison, when serving in his/her capacity as a liaison for the AOS, agrees that he/she
will at all times act and conduct him/herself in a professional and respectful manner, and will at all
times and in good faith represent the AOS in accord with standards one would expect of a
professional. - Liaison, when serving in his/her capacity as a liaison for the AOS, will at all times
fairly and accurately represent the interests and positions of the AOS, and abide by the AOS’ rules
and procedures, as conveyed or communicated to Liaison by the AOS Board of Directors. - Liaison assumes responsibility for his/her own acts or omissions. Liaison will and
does indemnify and hold harmless the AOS from any and all damages, claims, or injuries of any sort
or type which may result or be asserted by Liaison or a third party against the AOS or its Board or
agents as a result of the actions or activities of Liaison.
6. I consent to my name being publically listed on the AOS liaison page of the AOS website.
